idle problem

Default idle problem

my 92 arrord lx auto idles from about 900 to about 1500rpms what is the problem???
we changed the coolant temp sensor, cleaned the IAC valve, but when we pull one of the vacuum hoses (#21) it fixes the idle prob but smoke starts rolling out of the exhaust??
What can i do to fix the idle?

be sure the coolant is full; block burped thru the 12mm bleed screw at the T-stat housing.
Then you can adjust the FITV

we did adjust the HITV does it need to be tight or snug???
when we pulled the vacuum hose the surging idle stopped y is that?
