I put a temporary d16y7 into my ek project. Basically a OBD1 to a OBD2 car. At first my idle wasn't so great either. After F'ing up the IACV and a week of work I found that just properly resetting the car got it idling fine. Also understanding that the IACV on my engine is warmed by the antifreeze so it doesn't go smooth till the engine warms up.

try switching the map and tps plugs with each other, you may just have them backwards which would explain the rough idle and the safe mode low tach

are you sure that you have enough coolant? did you let it run up to temperature with the cap off to get out any air in the coolant lines, helps to squeeze the bottom radiator line as well, all the vacuum lines correct, if you unplug the iacv can you rev the past 4500?
