Default D16A9 overheated, why?

I have a civic 92 D16A9 - 1600cc VTEC automatic.
I face a big problem that the car after 4km running goes to overheated and the heat pointer stop nearly the midle of range. If I try to run with more than 80km/h the motor is overheated an the pointer goes directly to red area.

I disassemble the head and clean it also change the head gaskit. Also I clean the ratediator , change the watre pump with new one.

The response of car is very good no problem with its running, just overheated?

What can I do also and what check also....

please need help !!!!!!

Yes I resurface the head before change the gaskit

Also the fan is always turnned ON.

So, I bypass the thermostat and remove it, therefore the fan is ON directly with turnning the engine ON.
