someone probably left off the timing belt guide plates on the crankshaft when changing the belt

Then you fucked up on the alignment. If your crank pulley bolt comes out, and the crank pulley comes loose, it in terms comes loose. Time to change the T-Belt again since it's all chewed up.

How do I know it can all happen? I've snapped a crank pulley bolt at 5700rpm on my H22. I thought for sure I had fucked up my head.

simple fix... since you said you did the timing belt... obviously the tensioner is loose.. check out the belt, see if it has any wear from grinding..if not just tighten that tensioner up it may seem tight but it cant be if its floppin around good luck
