Starter Assembly blowing off of mount on motor???

Just curious, has anyone had their starter break or blow off of their motors? A friend of mine said it has happened to him and I find it unbelievable for a starter to blow right off.
The story I heard was one afternoon, he went to get gas and once finished filling his tank, he tried to start up his car. All he heard were a series of clicks, possibly those similar to the ones you hear when your battery goes weak. Then, After repeated tries at starting the car, he suddenly hears a whinning sound, constant with the turn on the key to the start position. Opens up the hood, finds the starter dislodged from where it should be bolted in.
If it is possible for this to happen, what would cause such a thing?

Well, the Dealer replaced the starter over 2 years ago. It had been starting up without a problem until that day. I kind of deleted that idea from possible reasons for that to happen.

True, but if that was the case, why would the problem occur over 2 years after it was initially replaced? It took 2 years time for a bolt to back out or fall out from its stripped hole and loosened the starter enough to blow out of its place?

Is this really possible?
It happend to my car once. The bolt was barely loose and after enough time had passed there was enough slack for the starter to jump and start skipping on the flywheel.
