I just did this on my '94 about a month ago. It'll work just fine but you'll have to get a different cat since the JDM header is longer than a stock '94-'99 manifold. I'd contact SMSP for a high flow cat and he'll get you one that'll drop right in.

The stabilizer bracket on the JDM ITR one won't line up with the stock '94-'99 exhaust manifold bracket though...you'd have to buy one for the JDM header. Inlinefour has these at http://inlinefour.com/25incolgas.html

This bracket isn't 100% necessary though. It's your call.

The bracket that you mention is for the JDM 4-1 ITR header. The 4-2-1 header uses the same bracket as the USDM header.

My 98 spec Mugen 4-2-1 header required the JDM 4-1 bracket.

I used the O2 sensor port in the header and coupled it with a JDM ITR catalytic converter. I had to change the bung for the secondary O2 sensor on the converter, as the JDM version was a 2-wire model.

This depends on whether you have an OBD1 or OBD2 car. Mine is OBD1 so I only had to deal with one O2 sensor. Basically, because the header is longer than the stock manifold, the O2 sensor for the JDM ITR header just goes into the bung in the collector (that is...the collector bung is in the place where the stock cat bung used to be). No extending of wires necessary...which is why I stuck with OEM Honda parts! I didn't need to mount one after the cat since my car is OBD1.

Hopefully that makes sense.

On a side note...the addition of the header and hi flow cat makes my car sound really ******' loud when I get on it. Honestly, I'm not all that happy with the noise level. I think I'll end up replacing my muffler sooner than later. Just be prepared.
