The level of complexity is relative. For me, it is a walk in the park; however, for my brother, it is hard.

How hard is it to but a B18C OBD1 engine in a car with no OBD (1990 civic). And what do you think of that engine in a 1990 DX sedan?

Just as they said above it's pretty darn easy, depending on how comfortabele you are doing the wiring. My swap was the first time I did it, and it was pretty easy. Look for blown90hatch, he makes good conversion harnesses .
