where is the IACV located?

Yes, you remove 2 M8 (12mm hex) bolts, and it will come off the manifold.
Be careful not to lose the rubber gasket - I suggest removing the gasket asap, once the iacv is off and putting it somewhere safe - dont forget to put it back.
There are also 2 coolant lines, and an electrical plug.

Have you messed with the idle adjustment screw at all?
If its too far in, go about the proper procedure and back it out to reset the idle to the proper speed

The IAC-motor could be shot. Try swapping out with a known good unit if you have a friend with a similar motor. The one on my B18C swap was stuck closed when I first bought the swap making it really difficult to start. After I cleaned it, it started working again, but it doesn't quite react fast enough engine is in high idle mode (warm-up). It causes the idle to pulse a little when the car is first started. It does works fine once up to temp though.
