Timing Belt rubbing valve cover?

Timing Belt rubbing valve cover?
Hello all,
Finally got my EM1 out of storage for the summer, and went for a little drive. Came home and noticed my white valvecover has a "burnt" spot on it where the timing belt is located. We pulled off the valve cover and side timing belt covers and some how the timing belt moved a good 1/4 inch or more and actually rubbed grooves into the side covers.
Needless to say I ordered a new timing belt, but what could have caused this so it doesnt happen again?

Sorry that im at work and car is at home. Plus what would you like a picture of? the rubbing on the valve cover? All you needa say is if there is to be a guide for the timing belt not to move or not. No need to be rude

Its rubbing on driver side. The belt has literaly moved off the cam gears a good 1/4" and seems like maybe there should be a guide to keep it pushed fully onto the gears?

But is there anything that keeps the belt straight on the gears besides the tensioner? I know some cars have a washer on the crank pulley that keeps the belt from moving off the gear.

This is the timing gear right behind the crank pulley. Is there to be a concave washer on both sides helping guide the timing belt?

The factory service manual diagram I posted above is for the 99-00 Civic Si B16a2 engine.
However, the diagram at Majestic Honda shows only one guide plate located on the engine side of the crank gear:
