Issue with crank pulley

Issue with crank pulley
1994 Civic EX, Stock D16Z6
So the problem was, my girl was driving it one day and she complained of a loud noise. So I checked it out. The car had somehow thrown the belts off the alt. and A/C. Weird. So I put them back on. Turned on the car and the crank pulley is wobbling like crazy. WTF. So I get it to my place, pull the crank pulley off and this is what I find.
The inside of the crank pulley is eaten away too. The key is pretty shredded.
So my question is, will it be fine if I just put another key on? I have another crank pulley to put on, and I'm going to pick up another key from the Honda dealer. I figured I'd get a key from Honda instead of the hardware store, because I don't want this to happen again.
Also, what could have caused this? The same Honda dealer did the whole timing belt service last April, right before my girl bought the car. I have Carfax records to prove this. I also changed the Alt about a month ago. I need some insight on this, this car has to work for my woman to get to school, work,etc. Thanks guys

My money's on the pulley bolt not being torqued to spec. See the dealer service manager about this. Since you have documentation that they recently had the pulley bolt off, I would insist they fix it or it is off to the BBB etc.

Based on the picture, the timing belt guide plate may have been installed backwards.
The lower timing belt cover has not been removed, right? If so, the front guide plate may be installed backwards.
Also check the rear plate when you remove the lower timing belt cover.
If the plate was actually installed incorrectly, call Honda and bring the picture as evidence.

Well while you guys were telling me all this good info, I was outside putting this beast back together. If the dealer screwed up, then I will be seeing what I can get out of them. A couple people I know work there, so I may have some inside help.
