grinding noise under high load
ok so i have a few questions, i have a 92 cx with a ls vtec swap, i have recently aquired a grinding noise, i ripped a cv boot a week before it started happening, so i figured it was the cv, but nope that wasnt it. replaced with a new one its still there, its only noticable in high load in 1st,2nd, and sometimes 3rd, only when im in it hard, if i take off normal shifting at 3-4k rpms i cant hear it. any ideas?

Re: grinding noise under high load
i have removed all of the heat shields. did that when i put my exhaust on, sounds like it is coming from front suspension area, and is really loud when i shift hard, i tried to video it but all you can hear is exhaust and intake.

Being that first and second gear is on the main shaft while third and fourth are on the secondary shaft, it very well could be a bearing on the main shaft, whether it's ISB or one of the back side shaft bearings. You said under heavy load at high rpm in first and second only. Or as NotARacist said, could be synchro or sleeve related.
Not sure how you are ruling all of these out without ever cracking open your transmission and inspecting and measuring is beyond me.
Too bad you don't have a dyno so you can load it up at 3K rpm without actually moving. Then you could stick your head under the hood and listen around.

At the end of the day, it's a safe bet that it's transmission related, and you should go ahead and crack the case. Buy/borrow/download a factory service manual for your car - it'll include the very detailed process of dismantling the transmission, measuring everything, and reinstalling it all again. Some basic hand tools, a few extensions, and a full set of feeler gauges should be all you need.

Re: grinding noise under high load
it could be the clutch adjustment, I know my third gear grinds almost all the time because its not adjusted right, its probably drivetrain
