#2 Cylinder dead. Found out why. What would cause this to happen?

Default #2 Cylinder dead. Found out why. What would cause this to happen?

I pulled out the plugs and the plug in the #2 cylinder looked like this. What could cause this to happen?

All of these plugs are only about 3 months old. I dont want to put a new plug in there if the same thing will happen again.

how do the other plugs look?

what is your loil level looking like and have you had to keep refilling it

is that black stuff hard?

Other plugs look fine. They are all only a couple months old. I burn oil really bad. I have put in a quart every week.
