It's just a Bosch sensor, if I'm not mistaken. I found this without too much trouble, I'm guessing any AEM dealer can get one for you:

Re: where to get AEM o2 Sensor??? (EGRhatch)
I'm curious if it was teh stock O2 sensor that died, or if you bought the AEM wideband O2 kit and that sensor died. If its the later, then you can do as suggested above. If it was the OEM sensor, you'll need the "kit", since wideband sensors need their own wideband controller.

Re: where to get AEM o2 Sensor???
Since it looks like no one ever really answered this, I will in case someone needs a sensor for their AEM/Innovate/NGK AFX on short notice.
It looks like Advance Auto can get them overnight as a Bosch 17205. They were used on 2005 Cadillac CTS as upstream O2 sensors, and they're listed as "Air Fuel Sensor" or something like that, as opposed to "O2 Sensor."
The down side is that their price on them is $112, as oppose to $71+ online.
Hope that might help someone out!

Re: where to get AEM o2 Sensor???
Maybe found an even better option for a Bosch LSU4.2 wideband sensor. The Bosch PN for this one is 17014, and my local Advance Auto has one in stock for $52. I'm going to go and check it out, and maybe pick one up for a spare, since I'm using AFR as a failsafe.
I'll report back if it is a match or not!

Dropped by Advance Auto to check that sensor out. CRAP! It's got a different plug on it than the one for the AEM. I forgot to look at the sensor itself to confirm that it still said LSU4.2 on it, but that end of it looked correct at a glance.
Oh well, it was good for a try!

Re: where to get AEM o2 Sensor???
After my wideband sensor crapped out again yesterday, I went and picked up the one from Advance Auto with the wrong connector on it (Bosch 17014). I wasn't sure I could just re-solder the other connector on it from the ones that are sold for the AEM (Bosch 17205), but since it had the potential to get me rolling again, I figured I'd give it a shot. Guess what, it seems to work perfectly, despite the fact that these sensors appear to have a significant chunk of electronics in the connector end.
The really cool thing about this is that you can get these 17014s on amazon or where ever you like for less than $50. 10 mins or so of soldering and you're rolling! Here's a couple of links for example:
