Explain how a timing belt falls off....
A few months ago, i was racing with a mustang, shifted from 2nd thru 3rd hitting redline, and a little bit of 4th. During the race, everything was smooth and ran fine. After i started to slow down, and tried to get back up to speed from a stoplight, the car stuttered and could move only about 5-10 mph at WOT in 1st.
I poped the hood and checked to see if any lines came loose and the usual stuff.
Checked spark plugs and they were fine. Then i went to see if my timing changed and i saw that my timing belt had slipped off.
Has this happend to any of yall? What could have caused it. I've never seen this happen to anyone else.

improper tension on the timing belt tensioner or failure of that item. If the tensioner fails then you would cause the belt to lose tension and it will become loose and fly off.

Do you have a cut valve cover, and are any of your timing belt covers removed? I'm assuming yes... Maybe the belt wasn't adjusted (tensioner) properly.

Check for the absence of the outer thrust washer on the timing belt drive sprocket. Those thrust washers are there to make sure that the timing belt can't move laterally.

Yep check that the thin metal washer is inbetween the pulley and the sprocket otherwise the belt will walk all over the place and eventually work itself off. I have seen this happen before luckily it was caught after the first startup, the belt was already half off.
