MDF mountings
Naa, you got it about as wrong as you can, although in another application, it may work.
The "mounting plate", [the piece of MDF that mounts to the door] is not round, it is the same shape as the part of the stock speaker that mounts to the door, even a little bigger, it has to fit into the dimple, and the mounting holes have to line up with the stock speaker housing mounting holes, [small nylon inserts in sheet metal, that you will remove]
The rest of the pieces are rings, all with the same inside and outside diameter, [inside diameter is the same as speakers mounting hole diameter, outside diameter is the same as, or a little bigger, the speakers overall diameter] the number of rings and thickness of the rings, will depend on speakers mounting depth and what you have with just the mounting plate, measure from the front of mounting plate to window/window guide in the door, lets call it 1.75", [your using 1/2"MDF for the mounting plate] your speakers need 3", 3"- 1.75" = 1.25", you can use a 1" MDF and a .25" MDF ring or a .75" MDF and a .5" MDF ring to get you the 1.25" you need.
Once you have the rings, you will stack them on the mounting plate, centering them where the stock speaker was.
Using a good quality wood glue and proper prepping of the gluing surfaces you will glue and clamp the stack together, overnight.
The rings will be outside of the door.
You will not skip the part about installing and countersinking "studs" in the mounting plate before you glue the rings over the mounting holes.
The mounting hole diameter, or close to it also has to be in the mounting plate, as you will need to reach through it to install washers and nuts, and a wrench to tighten them on to the studs, to mount the housing.
I also "paint" the the whole housing, [except the top rings surface, speaker mounting surface] with 3-4 coats of fiberglass resin, to waterproof them.

I think you have it now, although, not sure why you would cover the speaker with plastic.
Also, an inch or so of the back of the speaker would be inside the door.
You did not read the post closely enough...

The studs, machine screws that have been mounted to the mounting plates, [countersunk on the face of mounting plate and sticking out the back of the mounting plate 1/2 an inch or so], the studs will go through the doors sheet metal at the stock speaker housings mounting holes, [plastic insert removed] you will need to reach through the speaker mounting hole in the MDF housing to install washers and nuts onto the studs, that are inside the door.

I'm talking about the stock speaker grill (the one that came with the car, not the speakers). Is it suppose to be taking out.... I'm confused now.

^Correct. The MDF rings just allow the speaker to be mounted flush to the door. The spacer also allows the speaker's magnet to clear the window and whatever else may be behind it.
Once you get that all figured out, you just install your door panel back over it. Don't go too crazy with the spacers, as you don't want the speaker to hit the inside of the door panel also.
