got 2 problems. need help

ok so heres my setup on my teg. nothing crazy at all
94 rs. jdm b18c /intake /header / punched out cat/ exhaust chipped 94cx ECU. ( no A/C )
problem #1 ... took it to 8k, grabbed 2nd, and a bolt broke to some pulley and attached bracket on the side of the motor and was just bouncing around on the alternator belt and down in there.
i dont have a picture of the pulley and bracket, but it hasnt effected anything mechanical ( as far as i know so far ) all my belts were still tight and everything. im not exactly sure what it is. something for A/C maybe ?
anyone else had this happen before?
problem #2 ... had vtec just wired up on car( finally stopped throwing CEL for that ) , and header installed today ( not sure if relevant )
i tried to jump the harness with a paper clip to check the code, i made sure the paperclip was pushed as far in as it will go , but it wont flash it just stays constant ( doenst effect the way the car drives or anything to my knowledge while driving ) . it stops throwing code everytime i shut the car off. and comes on about after like 3 min. of driving it
any thoughts ?

a paper clip huh, that is different, i usually use a little two prong electrical connector or a small piece of wire. does the light come on when you are supposed to hit vtec, if so, it might be a vaccuum problem, i have never tried it, but try and read the code when the light comes on before shutting the car off.
