High pitched noise while driving or idle

High pitched noise while driving or idle
97 civic LX auto
For the last couple of days while I've been driving I've noticed a loud high pitched whine type of noise coming from the engine bay. It's loud enough to hear over the radio at moderate volume.
Sometimes the noise will be there even when the car is idling but I've noticed it is at the loudest at the top of the rev range in the first few gears. The noise seems more apparent at lower speeds and seems to come in waves... quiet louder loud, back down to quiet.
I'm stumped, any suggestions?
