Weird issue with caliper mounting bracket bolts.

Weird issue with caliper mounting bracket bolts.
Last year after finishing up my '90 DX-to-Si project I had the front/left top caliper mounting bracket bolt back out and fall off somewhere along the road which led to some exciting driving and parking-brake awesomeness until I could get stopped and have her towed home. I had a spare bolt, threw it in, and never gave it a second thought. Made sure both were tight and went on my way.
Fast forward to a week ago. I'm driving down the interstate and hear what sounded like a small part falling off my car, like a bolt. After that I would get a weird rattle when hitting bumps and the brakes sounded like **** in reverse. Turns out the front/left bottom caliper mounting bracket bolt fell out and the top one was loose.
Why does it keep doing this and what would be a good/safe way to make those bolts stay in place? I'll be ordering a new pair from Honda since they're only $0.60/each at majestic. Needed to order some hardware to fix my broken glove box anyway.

Looks like a bolt+washer or just the bolt, and the latter is the only one in the price list. It's also what I have sitting here on my desk(12x21). These knuckle assemblies are off an Si, and not even the DX had washers on it.
Threads on the remaining bolt and threads in the caliper mounting bracket look fine, or at least fine enough to continue using.

Re: Weird issue with caliper mounting bracket bolts.
On the page I linked there is a bolt + washer and next to that is just a bolt. There is no listing for the bolt + washer, but there is for the bolt. When they do this it usually means there was one or the other.
I've also never personally seen washers on the caliper mounting bracket bolts on any civic, integra, or prelude I've owned which is weird. I'm not saying they shouldn't be there as the diagram clearly shows a possibility of there being a washer.

Pull the whole caliper off, clean it up, grease the sliding part of the bolts, TORQUE them back in. Hand tight isn't a good idea. It will either be too loose or you will overtighten and snap one. They shouldn't fall off. And there is no washer for those calipers, just the bolt, the sleeve, and the grommets that serve as a shield against dirt.

Re: Weird issue with caliper mounting bracket bolts.
I checked all the threads again with the help of a flash light and everything looks fine. Even the remaining OEM bolt threads are fine.
I bought two brand new bolts which are a MM or two longer than the oem ones, some brand new lock washers, and everything is put back together. I highly doubt I'll have issues now that there are lock washers installed but I'll check the bolts every few days/week a couple of times to be sure.
Bolts on the front/right are still fine. I guess this front/left caliper mounting bracket is just an *******.
