I think this is down to cheap after market pads/shims, myself. I'm not sure if the shims used on my car were the originals (re-used) or aftermarket.

One other thing I'd just mention- since this could be vacuum related, when I pump the brakes quickly, the idle speed increases up to about 1200 rpm, then drops back to normal.
