Default M2b4 issues please help

Just built my h22 12:1 complete balance shaft delete and a ported head. However immediately after (aka within the first 100) miles my speedo went out then it popped out of gear wouldn't go back into gear but I checked and the cv axle is destroyed the flywheel teeth are chewed up and it chewed up the starter then to top it all it smoothed out the grooves in the LSD and put a hole in my case.

So any ideas or advice would be much appreciated!!! I was thinking I would patch the hole get it cleaned, put the carbon synchro tech rebuild and replace the diff with a quaife lsd. Then a lightweight flywheel stage 3 clutch new starter and upgrade to a yonaka driveline. But I am at a loss as to why this happened and have no idea how to prevent it in the future so please help me it would be much appreciated!!!

The M2B4 will require the top case to be replaced along with the countershaft and ring gear if you want to install an aftermarket LSD, but it sounds like this one is beyond repair. I will PM a quote for a complete trans.
