Weird stalling problem

Weird stalling problem
I think this is somehow vacuum related.
When I start the car, cold start, the RPMs will rise to around 1500...stay there for a few seconds then the car dies. If the engine is warm, theres no problems on start up.
When Im driving, if I come to a quick stop where I press the brake rapidly and the clutch, the car stalls.
If I press the clutch in from say 3000 RPMs and let it drop, no stall...or if I break easy it doesnt stall. Only when I break a little harder the car will stall. Any ideas?

And to add, not sure if my vacuum is good or not.
At idle its at around -280mmHg to -300mmHg and at 3000 RPMs its at -550mmHg when holding it steady (not reving)
Does that sound right?
