Default timing belt slip

I just got my head back on my ls integra after having to fix the head because the timing belt slipped and bent my intake valves. And it did it again after less than 10 miles. so now I have to fix the head again but what else do I have to fix so it doesnt happen again? I know the belt was as tight as you can possibly get one and the tensioner was tight too. Can anybody shed some light on what might be happening?

improper cam to cap clearance
too tight of valves

you say timing belt stays tight right?

not to sound like an *** but you did put a new belt and tensioner on right?
if you did id replace the cam gear possibly, i dont see how it would slip brand new unless it wasnt tight
