Default prelude timing belt

I just did a timing belt water pump change on my non vtec prelude. The timing belt keeps walking off of the cam gears. Nothing seems to be warped, everything is tightened and torqued to spec. Any ideas?

Thanks

probably have to retighten your belt tensioner. First turn the crank counterclockwise until it takes off the slack on the belt, then tighten up the tensioner bolt. You may have to lift it up.

The timing belt seems to be correct, I have done it like the manual said, and then by pulling on the tensioner to make the belt tighter as well. When I turn the engine over by hand the belt stays put, but when it is turned over by the starter it works it's way off.
