Default my window keeps falling off track

I have to fix it at least once a week and its driving me mad! Does anyone know, has had this problem? i've checked everything and all I see is this little rip in the rubber.

Rollers are off and need to be adjusted. Happened to me once when I had my civic.. Got it fixed at the dealer (under warranty)

this happened to me before...i just took out the whole assembly and made sure everything was on right...it never came off again

I think the rollers are off. I'm an expert on taking my entire window apart not, but I hve no idea why it keeps falling off. Let me check this roller thing. I no longer have a warrenty
