Default H22 Timing Problem Car just shut off

So here goes my long problem. I ate through one timing belt all together. So recently I replaced the timing belt and the adjuster arm on my 94 H22. When I started it I noticed that the timing belt slid out off the cam gears approximetly 1/8 of an inch. So I bought a brand new auto tensioner and installed that. To no avail, It did the same thing again. So I took it for a harsh run and noticed that the belt whent in again. After another harsh run it came back out just as far as before. My wife was coming back from getting gas and the car just died on her. She said she heard a noise to the effect of a blow of valve. So I checked the three things needed for a car to run. I had spark but it was not as powerfull as it should be, it was kind of only half assed. I checked fuel and I have fuel coming off the fuel filter, and off the fuel rail. The dizzy apears to be fine, i just took the cap off and all seems to be normal. I could not check compression where I was at (in the middle of now where). We noticed that when we put the tranny at TDC the cam gears were not alligned like they were suppose to be and that dizzy was firing at the #3 cylinder. It appears that the timing slipped. But how could it do that. Brand new timing belt, balancer shaft belt, adjuster arm, auto tensioner and the tension was tight and still is. And yes everything was timed when I put the belt on. I drove it about 40 miles and 4 could runs down the strip, then it decided to die. Any one have any ideas? I am totally stumped here.

What about those dished washers that are supposed to go on either side of the crankshaft timing sprocket?

There is one dished washer that goes close to the block on the crank shaft and that is there. I think the timing belt problem is fixed, but I can not figure out why the car wont start.

Don't know if I can explain why it slipped, if you had the tensioner & everything in there correctly...
