clutch slipping/problems

My clutch is going bad, and Id like an input as to some possible causes.
The symptoms:
-Clutch pedal engages very close to the floor, lost some pressure in it
-Impossible to get into or out of gear when the car is on
-If I start the car with it in gear, and keep my foot on the clutch, the car lurches foward like im engaging the clutch
thats pretty much it. I added some clutch fluid to the resevoir and noticed a little difference in pedal feel, and the car drove for about 1/4 mile, then it started acting up again
Any ideas on what to look for when I open it up? TOB? Pressure plate? Leak/bubble in the line?

yea, if it's a hydro clutch (which i'm sure it is), check your fluid. if it's low, check for leaks. fix any broken parts, bleed the clutch cylinder, and make any necessary adjustments.
