Default b20v over heating

Sometimes when I drive, my CEL would throw for Engine Over Heating Temp.
I would open up my s300 v3, and temperature would read at 200-219
but sometimes my needle would stays the same even when it's reading 219 degrees. And yet sometimes the needle will go up. So i'm not sure if it's HEAD GASKET or not....
My reservoir is full and sometimes the reservoir cap would get all wet, which I don't know why.
When I open the radiator cap, there's air pressure and lose about 1 quart water...I would top it off and bleed again and same thing, air pressure and lose 1 quart.

This has been going on maybe about 3 months. Some days I would drive a whole entire day and this problem wont even show up.

Can it be headgasket? Reservoir cap???

Stuff I've done so far
-Checked fan, it kicks
-Thermostat
-Ect sensor replaced

compression test all and it reads 230 240 240 250... 4 3 2 1

Your "overflow/expansion" reservoir has a full hot and full cold level line on it, it should never be full, [to the top].

I would start by replacing the rad cap.

When engine is at running temp. there will be pressure in the rad, around 15PSI, depending on cap, if you remove the cap, coolant will blow out, a quart is about what you would lose, there is no pressure in the reservoir, [or there should not be any].

You may also want to replace the water pump. 94

I think the water pump is a rather silly suggestion. Failure of this component is extremely rare and generally only a weep hole seeping. Also, it does not come and go, sometimes ok sometimes not. You may have some issues with gasket seal, 219 is too hot for normal running conditions.
