Default Oil in coolant - 91 hatch Si

I have black sludge in my coolant resevoir, i assume its oil. My car has spit out a few puffs of white smoke out the exaust also. I've searched and searched but couldnt find any info on this.

What likely causes this? Blown head gasket? What needs to be done to clean it out if it even needs to be more then drained?

I bought a helms because i know i need to pull the engine apart, but could use any info so I know what to look for going in.

Anyone have any info?

What you have is a seeping, on the verge of blown headgasket. The D series with its composit head gasket iss notorious for this issue. Honda issued Service Bulletin 97-047 just for this issue. In it a new metal headgasket , headbolts and a revised torque sequence is described.
