what can lead to failed ring-lands?
if the 2 small rings failed id say the best cause is the ring was too big for the hole. not enough clearence.
if the wavy ring failed most likely improper installation. as in the 2 ends were overlapping when installed.
never really seen a problem with them failing except when they sit for a while and freeze up.

Are you referring to only the oil ring only in this statement?
