5th gear pop out, 91 Teg. WTF?

My sisters 91 teg (stock) pops out of 5th gear. I have heard of this on SR20 SER motors, but never on a Honda. Any ideas of what the problem might be?

Your tranny fluid might be low or old. Change it

any other ideas?? same thing is happening to my bro's 99si... we just changed the clutch and resurfaced the flywheel
