hmm...here is a thought
Add a set of air horns to the alarm.
Very loud...very effective

I once had a customer request 4 seperate sirens for his alarm...
one under the rear bumper
one inside the car
one in plain view under the hood
one hidden under the hood

you can add as many sirens as you want, you just many need to add a relay to make sure it doesn't burn up the alarm's output

but airhorns are definetly the loudest sirens you can add to an alarm

on some sirens there is a little black wire that looks like it just loops from the siren and back into it, snip that wire and tape them up seperatly, it'll be loud as FUgg.

A Mini Piezo siren works good for getting Attention. But yes the Air Raid Sirens are the best thing out there for getting the Car Noticed. We have two of them on our Si and its Fawkin Loud.

I was thinking of a similar problem: when an alarm goes off, who actually looks? Does it actually attract attention or scare off a theif away?

That's what lead me to installing a police siren as a backup horn. EVERYONE will look if it sounds like the cops are in the parking lot wailing away. The buzzer horn feature (the one that ambulance/police/fire vehicles use when going through an intersection) is my favorite to use! Of course, the siren can only be used when the vehicle is not moving. Check with your local laws if you're considering doing this.

The horn is a 200 watt piece that I found on eBay for under $40. The "head" or amplifier a police officer friend found for me. They go for around $20 to $50 depending on features. Mine has a PA that I haven't gotten around to using yet.
