get a new cat if you don't have a new one allready.
try some GM top engine cleaner do a tune up your nearly pass soo it shouldn't be hard to pass

Hey! It's not your cat...run the Guarantee to Pass Emissions Formula in your gas tank..you can purchase it from any Auto Store. if your cat was the problem, your CO and HC would be off the charts... Since our cars don't run EGR valves, you might want to try and do a tune up, and run that stuff i mentioned earlier...

Magic elixirs only band-aid the problem temporarily.....Fix the REAL problem and you wont have any future problems....Some laws were set up for a reason...Although I would hate to live in Cali b/c of smog laws, it does have some of the nastiest air and dirty cars isnt helping any bit.

bossman what are you like in the 5th grade? wow i forgot you're da man..
anyways.. dude with the cat problem..take it off and Check it out...if the internals of the cat are broken, then replace it, also check it and see if you see any blockage..hold it up to a light and see if you can see through it. If you can and there is no damage to the internals, don't swap it...
It's gonna be a pain in the *** to take it off, bolts and nuts will probably be rusted. I suggest getting replacements bolts before you start just in case, get the gaskets also... you don't want to get stuck. Likely the bolts that will be rusted will be the three connected to the exhaust pipe..(rear part of cat). Spring bolts should be fine.
bossman suggested replacing it, but that's 150 some bucks if money is no object all is good..or if you're like the rest of us and can't dole out 150 without consideration you can go out and spend 8 bucks and one weekend and use that emissions liquid i suggested earlier. read the instructions on the box..
either way It's your call...worst case scenario if you don't pass you're out 9 bucks but you still get the benefit of it cleaning out your fuel system.
Post your results when you're done...and your emissions record and what route your went.

But I have been to the ref about 5-7 I cant remember how many times soo and I know what I am talking about if you want to preach but at least preach the right information.
If your cat is 13years old it should be replaced if your failing emissions.
FYI: I had an old cat like yours getting a new cat reduced nox and HC nox buy 350ppm and hc by about 70ppm.

U just barely failed NOx, so i wouldnt replace the cat. It doesnt appear that it is totally at fault. It seems that if that is the only thing that failed (assuming your ignition timing is dead on) then run some of GM's "top engine cleaner" to clean up some of the carbon build up u may have. NOx is caused by too high of combustion chamber temperatures. What causes those high temps can be a lean condition, too far advanced ignition timing, carbon build up creating higher compression (thus more heat), etc.
