Do a compression test.
If he has low compression in one cylinder, he could have a bent valve.
Hondas have loud valve tick. Thats just how they are.

Is it ticking real loud?
If you suspect a bent valve, first things first, do a valve adjustment, see if they are all in spec. Many times even with a misshift, the valves just get horribly out of adjustment and not bent.

Idle will be crappy
Also you can do the dollar bill trick. Put a dollar bill in front of the exhaust and if it looks as though it's sucking in than you may have bent valve(s)

If I was you I would just go get a valve adjustment. I had the same problem with my car and it knocked pretty loud.
