When changing the rear pads, do you adjust the threaded rear piston so that it almost engages the brakes. Maybe backed off a half turn. Or do you spin it all the way in and leave it?

someone correct me if I did it wrong, but I turned mine all the way in. Once everything was back together, I pumped the brakes till I felt the pads firm up.

Yes, just push it all the way in. When you step on the brakes a couple of times, it will push out up against the pad.
