is it my pressure plate or clutch master cylinder

Clutch isnt fully disingaging shutters when in gear and clutch pedal is to floor.. gears are not going in sometimes and they are grinding... everything is telling me that the clutch is not pulling off of the flywheel...
When driving in gear it sounds and runs fine just when you press the clutch in are there problems
so is my pressure plate shot, needs adjusted or does it sound like the clutch master cylinder?
When it first happend i thought the clutch fluid was full but it just left a shadow once i opened it i found it dry.. i filled it bled the lines and it didnt fix... this is why im thinking the clutch master cylinder went out.. but someone is telling me they think it is the pressure plate
any experience with this problem?
i want to get some experianced answers before i go pull the tranny off

check the hose were it attaches to the steel line on top of the transmission. my line started leaking around there 2 months ago and i had to replace the hose. it's around $35 from Honda and only honda sells that hose... edit: majestic has it for about $10 less.

i had to replace # 16, it was leaking where the clamp went around it to hold it secure against the transmission.
the guy at the dealership said it's common for our cars and accords to have that go out after about 8-10 years.

thats fine and all but the clutch master cylinder is full of fluid now and im still getting the problem..
that might be the source of the leak but it wont fix the big picture
