Stumble after 2 days of boost

Default Stumble after 2 days of boost

ok, 2 days after stock boost, the car starts to stumble when it sees boost,,,,anything under 2500 is fine no stumble....Whats wrong??
All vacume lines and charge pipes are connected,,,no leaks...

Heres the set up:
ZC VTEC (JDM D16Z6)
Greddy TD04 Kit w/ greddy type 31 FMIC
DSM 450 cc injectors w/honda resistor box
VAFC hack (-40% across the board)
Greddy SP exhaust
Airmass cat


Codes that were pulled:
14-iacv (taken care of)
16- Fuel injecto system (dont know whats wrong here)
22- VTEC oil pressure switch (taken care of)
41- O2 sensor (brand new)

So what do you think could be the problem?? I am only running @ 5psi,,,I am confused, could it be the o2 gone bad??? becuase it only has like 4k miles on it if not less

ok dont flame me for a dumb idea, but the dsm injector and the resistor setup... how did you wire it up?

were the injectors cleaned and whatnot when you got them?

I wired up the set up by the diagram on the Hondata site, the injectors had 40k on them and were cleaned.... My big question is, could it just be the o2?? my A/F is stuck on rich so i imagine that the injectors are fine, plus all the plugs look the same.
