try spraying in some throttle body cleaner. the butterfly may be getting stuck.. if that doesn't work, a new cable's only 25 bucks or so..

My old one used to do that. Make sure the spring on it is good because if they aren't strong enough it will stick open.

try spraying some wd-40 on the throttle cable linkage too, where the throttle body connects to the throttle cable. it may be getting stuck there too.

also, check the vacuum line to the dashpot (if your car is so equipped). This, in combination with the metal clips that route the throttle cable and the weak throttle spring, caused the same problem. I ended up zip-tying the cable loosely to the factory cable routing clips and adding an extra spring to the throttle cam to return it. Oh well- it works now.

ditto... i wonder if it's a problem using the longer DX/Std throttle cable with the MPFI intake manifold and tb.

happened to me in the middle of winter. like -20 degrees just started my car and after i gave it some gas to back up the cable just stuck.... shot straight to 7000 with the engine dead cold before i turned the key off.

happened like 3 more times this winter. hasnt happened since it warmed up though, and the d15 is still going strong

You have to reroute the DX/STD cable for it to work, I've seen people basically run it and wrap it around the engine mount once and then to the throttle body. When I did my swap I got a throttle cable from an Si at the junkyard, screw ghetto rigging stuff up.
