I got confused too, I left one area blank accidently, so I resubmitted filling in only that area, thinking it would include the rest, but it didnt. So, I ended up submitting 3 times. Oops.

&lt;---Stupid non-ITR owner.

 

Sorry for the confusions guys. When you fill out the form and hit submit, it will come up with your entry in a table at the bottom of the page for you to review it and this also verifies that your entry was successfully placed into the database. You only need to submit once, then check the table at the bottom to review. If you made any mistakes and have changes to your entry, just email me at gdunn@gmdinteractive.com.

Also, please make sure you read the information on the cd. I need an accurate count of those who will be purchasing in order to calculate final pricing on the duplicaton, etc.
