Default clutch question

Gotta little problem... Ive got and 97 civic ex, and the slave cylinder piston on my clutch isnt pulling back enough for the throw out bearing to fully disengage... it 'chatters' on the pressure plate forks anyone encountered this before? Its all self adjusting, (except for the pedal, which I've done) anyone have a clue where I should go next? Replacement? if so, what?

thanks!
