Default DISC BRAKE conversion

I'm planning on converting my drum to disc brake off Integra to my civic hatch 95. What exactly do I need to get from the Integra to complete my conversion? I heard about the proportioning valve that I need to replace as well. Where is the proportioning valve located at. If it is the one by the firewall behind the engine, how come there's only 4 lines coming out of it. The civic has a total of 6 lines. 2 from the master cylinder going into the 2 inlet ones and going out into 4 lines. 2 for the rear, and 2 for the front. Can anyone please help me what am I missing here? anyone with the experience please give me some info. Thank you in advance.
