Default Idles in neutral 1500rpm and now has cylinder 1,3,4 misfire

Hello everyone I have a 2000 civic manual transmission. For about 2 months it started idling high you can hear it clear so I put my scanner live data and saw that when in park/neutral it idles 1500rpms. When I get to a stop sigh or red light I downshift and that brings the idle down for about 2 seconds then back again at 1500rpm eventually 2 months later I got a multiple cylinder misfire. My car does not overheat or does not have a vaccum leak. I checked my fuel trims that are perfect. What would cause this ? Thank you all.

Clean the IACV and do an ignition system tune up. Then go from there.
