Is it the engine rattling, or could it be a heat shield or something related to the serpentine belt?

Its the engine. We also have a 2016. We changed the oil in the 2016 using 0-20. Initial startup had a rattle that I know for a second or two. Oil not circulating until filter is full etc, The 2013 makes the same sound after it sits a few hours then started. Tried some 5W30, no change. Going back to 0-20.
If its been started, re start makes no noise. If it sits over night, makes a helofa clatter for the first second or three.

I have had a couple of cars over the years where there was pronounced start-up clatter; I ended up discovering it was the oil filter's "Anti-Drainback Valve" (ADV) which was the culprit. The thing is, some engines have an integrated ADV; many rely on the oil filter for that function. My advice would be to try a different brand filter and see if that helps.

This sounds a lot like the VCT Solenoid/Actuator, there is TSB for the 12-13 CRVs on it, I don't know if they extended it to cover any of the newer CRVs. But based off your description its seems to match, can you tell where the sound is coming from if you have the hood open and are standing in front of the car listening?

Filters are cheap though, so it doesn't hurt to replace that to make sure.

I put a Honda filter with no change.

Going to listen with hood open. It needs to sit a half hour or so.

Sounds like "Shot engine " to me.

If you haven't already I would look into the TSB I mentioned in the above post. Don't want to jump to the conclusion of a shot engine before looking into a known issue. The sound that VCT actuator makes can be nasty sounding.

Start-up clatter does not in any way equate a "shot engine".

Replaced the cam shaft gear. All is well, thank you for putting up with me.

Took a couple hours, part was $115.00 local.
