What did you change between the two tests? The first one was running obscenely rich. The second one is probably also too rich. You should look at fuel pressure. Check that the engine warms up to operating temperature so it goes closed loop, and that the ECT and IAT are within spec measured with an ohmmeter. Also inspect the spark plugs, if one is different from the others (like black), that cylinder may have a bad injector.

Had a similar problem years ago, I had my injectors cleaned/rebuilt locally and it solved the problem. Best guess is that they were so old and gummed up that they probably never fully closed when they were supposed to.
check the cap good, i had a rotor screw come out once and it wasted the pickups
also not hating but where you getting your maps from, there are some good guys in the ecu forum, you might see if they can take a look at the test results and see if they think its a map issue,

Flash a totally stock map, or remove the jumper so the ECU runs stock firmware instead of from the chip, or use a stock ECU.
Things I've asked you before: Coolant temperature proper (193 degree thermostat installed and working)? Valve adjustment good? Any signs of misfiring? Any difference in color between the spark plugs?
