Default Timing belt walked, how to fix?

So I started my fresh rebuilt motor and the timing belt walked to the side quickly. Here is a photo:

What caused this? Any ideas? I have the thought that the belt is not centered on the crank, but with the motor in the car and lower timing belt cover on, I can't tell. Or is my tensioner just loose? Suggestions are welcome. I'm actually just excited that the car started right up and didn't puke smoke out the exhaust immediately.

And yes, it hit the cover and burned through it immediately. The car ran for MAYBE 15 seconds before being shut down.

Butterfly washers at the crank.

Take the crank pulley off and check orientation.

Then redo the tensioner.

Alright, thanks. I'll look into removing the cover to get a look at everything.
