Default 96' civic/b18b1 swap - Stupid Code 41 Won't Go Away

I have a 96' civic hatchback with b18b1 swap. For my primary o2 sensor, i cut into the o2 sensor wires to extend them to reach the plug (Yes, I know, dumb idea and will never do it again) and after the car ran for about a day or two my CEL popped up and it was a code 41 (primary o2 sensor heater) which is when i realized i probably messed up the o2 sensor by extending the wires. So i replaced the o2 sensor, got an extension harness, and reset the ecu. Code went away, then two days later the same code 41 came back. Did i cause a short? How can i check? Any input is greatly appreciated. Thank you
