Default 4th gear issue in high rpms

I have a stock b16 trans on a b18 motor. ACT stage 3 6puck clutch that is about 6 years old with 25K-30K miles. The clutch doesn't spin at all. Shifts perfectly fine at low speeds and even works fine shifting into 4th from 3rd gear(at the low speeds). The main issue is sometimes grinds from a 5th to 4th on the highway(50-65mph) and when I am in boost/full throttle shifting from 3rd to 4th, it seems as if it will not go into 4th at all. It just hits a level or something and wont go in on a quick shift. If i granny the 3rd to 4th shift it is ok but cant at all shift faster then normal. Again this is the only gear I have any issue with on full throttle. any ideas? I dont want to spend $1000+ on getting it rebuild or spending $750-900 on a used trans.

Open it up and examine the suspect areas. If you need second opinions feel free to contact us or post pics here and we can likely tell you what would be the best fix for your budget.
