What would cause the Timing belt to slip off top gear?

Default What would cause the Timing belt to slip off the top gear?

I just replace the timing belt in my '89 accord Lxi.
We sync'd everything correctly. I was idling smoothly..but we saw a little black smoke coming from the timing belt cover so we shut it off. We opened the cover and the belt was working it's way off the gear and rubbing on the cover. We didn't replace the tensioner..is that the culprit? Is it not being tensioned correctly?

Yeah, sounds like it wasn't tight enough.
