b16 trans pops out in 5th

OK so I'm having a issue with my b16 trans popping out in 5th. Pulled trans out and inspected everything in the case and all looks brand new. But while crushing in in 5th it just pops out into neutral. I can feel the shifter move when I let off and on the gas. No other gear does this movement. What could it be causing this. Fluid level is good with honda mtf. Axles appear to be all the way in even though they are advance auto axles. I have innovative street mounts. Everything shifts like butter no grinds not even in fifth. I'm at this point lost as to what can be causing this.

i didnt inspect the linkage bushing just a quick look and they appeared fine with no obvious signs though they are enegry suspension stablizer bushings i have had in there for years so they made need replaced. same with the mounts no obvious rips but they are only 2 years old

That's what's bugging me and my friend. He works on trans and hasn't been able to figure it out. We did replace the fork just to be sure there wasn't a slight bend that we couldn't see. Now we are puzzled. Could it be a pressure plate issue maybe

When we put it the trans in it pop out of fifth. So we took it back out opened it up inspected put back together and it still was popping out. The first time around when I drained it the fluid was milky which I think was from when I hosed it off the first time lil bit of water got in it. So I thought that may been the problem. Filled with new honda fluid and still doing it. I'm having my mechanic open it back up soon once the weather gets decent around here again. There weren't any left over parts after reassembly. I would hope my mechanic would have noticed the missing the spring though if it wasn't in there from the get

We have no clue as to what the issue is so we are taking everything to do with fifth gear out of my gsr trans and putting it in the b16. We changing the shifted box inside as well. Is this a straight forward swap? I know one of the shafts I believe the counter shaft is the same part number what all would be needed to do if I were to put the main shaft in?
Make sure you guys didn't put the 5/R sleeve upside down, too. The Reverse side's teeth don't have a pent-roof cut to them to keep them locked in gear under load. That and make sure the synchor spring on 5th is in good shape.

How can I tell if it is in backwards or is a obvious thing? I know nothing about trans work so I wouldn't know from looking at it but my mechanic will see if it is or isn't
OP the teeth will be strait on the R side of the slider.
