Default Slipping Clutch? How can you tell?

What are some symptoms of a "slipping" clutch? My car tends to stutter slightly when engaging the clutch in first and second gears. It's only present when the car has been driven a lot that day. It won't do it in the morning but more towards the end of the day when I've put 20-30 miles on it. Is this a sign of a failing clutch or does it just need adjusting?

Usually the symptoms are that the car wont start moving until you goose the throttle a lot. Itll do it especially when your going up hil.
