Default '92 Civic VX won't pass emissions - high HC reading and NOx

Had my car inspected and unfortunately didn't pass

Here's part of the report showing what failed and what passed:

Things done on the vehicle recently:
5 wire O2 sensor NGK - June 2011 241k miles
Full tune up (NGK V-Power spark plugs, OEM Honda spark plug wires, OEM fuel filter, but no distributor cap and rotor replacement - June 2011 241k miles
Timing belt job with water pump and tensioner (all OEM) - 241k miles
A new cat was installed when it wouldn't pass the last time, but that was in May 2007 at 157k miles

What do you guys think could be the issue? Is it possible that a cat could fail with a little over 100k miles on it?

Most likely the cat as most aftermarkets dont last over 100k. I would try seafoam, and clean out the throttle body, and egr valve.

A common cause for high HC numbers with low CO numbers is a misfire caused by the engine running LEAN. I'd be closely inspecting for vacuum leaks and fuel system faults. Also, since the VX has an EGR system there are 2 other ways your car can run lean. If the EGR valve is stuck open, the constant flow will make the whole system run lean. Another common problem is when the valve itself is fine, but a few of the manifold ports get plugged with carbon. The result is all EGR gas goes through the unplugged port, causing a lean misfire on that cylinder.

Catalytic converters last a long time IF the engine is running right. A poorly running engine can kill a cat quick.
